Terms & Conditions

These Terms and Conditions ("Agreement") govern the provision of services by Claytree Therapies (referred to as "Company", "We", "Us") to the client (referred to as "Client" or "You"). By engaging the Company for services, you agree to be bound by these terms.

1. Scope of Services

The Company provides specialized services in Northwest Arkansas (NWA) centered on rehab, performance and recovery.

1.1 Physical Therapy (PT) Services

  • Provider: Provided exclusively by a Licensed Physical Therapist, Neal Clay III.

  • Definition: Services include comprehensive clinical evaluation, diagnosis, manual therapy, functional movement assessment, and prescribed therapeutic exercise for the treatment and prevention of physical dysfunction and injury.

  • Objective 3D Analysis: The Diagnostic Movement Blueprint includes the Zero-Radiation 3D Biomechanical Assessment. The data derived from this assessment is the intellectual property of the Company and is used exclusively to inform the Client’s treatment and training plan.

1.2 Coaching and Performance Services

  • Provider: Provided exclusively by a Certified Coach, Natalie Gentry.

  • Definition: Services include customized training program design, performance optimization, neuroscience-powered coaching, and ongoing support for athletic goals (Running and MTB). Coaching is distinct from, and is not a substitute for, medical advice or Physical Therapy.

2. Financial Terms and Payment Policy

2.1 Cash-Based (Out-of-Network) Model

The Company operates as a cash-based, direct-pay service. We do not accept direct payment from any private insurance company, Medicare, or Medicaid. Full payment for all services is due at the time of booking or prior to the commencement of the session/package, as detailed in your Service Agreement.

2.3 Superbill/Reimbursement

Upon request, the Company can provide a Superbill, which you may submit to your insurance company for potential out-of-network reimbursement. The Company makes no guarantee that your insurance company will reimburse any portion of the fees paid.

3. Scheduling and Cancellation Policy

3.1 Concierge Service Area

The concierge service is limited to a

20-mile radius from Bentonville, Arkansas

of the NWA area. Sessions requested outside this range may incur a travel surcharge of

$50

3.2 Cancellation and Rescheduling

  • Due to the time reserved for in-home concierge sessions, the Company enforces a strict 24-hour cancellation and rescheduling policy.

  • Cancellations or rescheduling requests made less than 24 hours before the scheduled session time will result in the forfeiture of the session fee (100% of the cost).

  • Clients must communicate cancellations via PHONE CALL OR TEXT

3.3 No-Show Policy

If the Client is unavailable for a scheduled in-home session, the session will be considered a No-Show, and the full session fee will be charged.

4. Client Responsibilities and Agreement

4.1 Adherence to Program

The Client agrees to follow the prescribed home exercise programs (HEP) and training plans to the best of their ability. The Client understands that failure to adhere to professional guidance may negatively impact recovery or performance goals.

4.2 Medical Disclosure

The Client agrees to disclose all relevant medical history, ongoing treatments, and medications to the Physical Therapist and Coach prior to beginning services.

4.3 Limits of Coaching

The Client acknowledges that Coaching services are for performance and training purposes only and do not constitute medical advice, diagnosis, or treatment. Any pain, injury, or medical concerns must be addressed by a licensed physician or physical therapist.

5. Liability and Disclaimer

5.1 Assumption of Risk

The Client acknowledges that Physical Therapy, training, and exercise inherently involve risks, including the risk of injury. The Client voluntarily assumes all risks associated with participation in any program or exercise recommended by the Company.

5.2 Release of Liability

The Client agrees to hold harmless and release the Company, Neal Clay III, and Natalie Gentry from any and all liability, claims, or demands for personal injury, property damage, or wrongful death resulting from the Client's participation in the services.
